2009-10 FIRST Consent Form

Before your team can participate in a FIRST competition, students, mentors and volunteers must fill out and submit a FIRST “Consent and Release” form. These forms are your team’s “admission ticket” to the regional competition.

Every FRC team must submit a team roster and consent forms to the Pit Administration Supervisor at the initial regional event in order to complete the team’s registration and be allowed to participate in the regional.http://www.usfirst.org/roboticsprograms/frc/content.aspx?id=8128

New this year, FIRST is offering student team members the opportunity to register their consent form online. For students under the age of 18, this requires two steps. All students should register by creating an account here: https://my.usfirst.org/frc/goteams/site.lasso

Upon receipt of an email from FIRST, the student needs to logon again to the website, and complete and submit the information requested. At the same time, for students under 18, a parent or guardian will also receive an email from FIRST.  A parent or guardian will need to logon to the consent form page and sign the online consent form. Each FRC team’s main contact is able to track student consent form registrations via the “Team Information Management System” or TIMS. This student roster must be printed out and brought to the regional event. The names of students not registered online should be added to this roster, as should the names of all team mentors.

Team Mentors: Team mentors cannot register their consent forms online. Please use the 2010 Consent and Release Form here: http://www.usfirst.org/uploadedFiles/Community/Asset/2008-2009%20FIRST%20Consent%20and%20Release%20Form.pdf. Write in the names of team mentors on the student roster form printed from the TIMS.

This paper form should also be downloaded and signed by students who are not using the online consent form system.

All printed – and signed -- consent forms by students and mentors should be included with the roster of the entire team and brought to the Pit Administration Supervisor at your initial regional competition event. This will complete your team’s registration at the regional event.
