2012 FIRST Consent Form
Before your team can participate in a FIRST Kickoff or Regional Competition, students, mentors and volunteers must fill out and submit a FIRST “Consent and Release” form. These forms are your “admission ticket” to FRC events. In 2012, students and mentors should file their Consent forms online.
At both Kickoff and at the initial Regional competition attended, every FRC team must submit a current team roster listing all students and mentors, as well as attaching paper Consent forms for anyone who has not completed the online Consent form.
At Kickoff, the roster and Consent forms will be delivered to the Coordinator. This will allow teams to receive the paperwork needed to collect the Kit of Parts.
At the Regional Competition, the roster and Consent forms will be submitted to the Pit Administration Supervisor. Information and instructions are posted here: http://www.usfirst.org/roboticsprograms/frc/content.aspx?id=8128.
FIRST encourages all student team members to register their Consent form online via the Student Information Management System (STIMS): https://my.usfirst.org/stims/site.lasso
The team roster must be printed out and brought to the regional event. All names of students and mentors should be listed on the team roster. Any student or mentor not listed should be added to the roster using the space provided at the bottom of the page. All paper Consent forms should be stapled to the team roster.
